Yep, the gift of pain is the key. Normally this perk works against killer, because genrushing while injured is one of the most effective strategies available for survivors. However, Oni is that one killer who would absolutely love survivors doing that. His power directly capitalise on survivors being injured and allows him to punish them for genrushing in that state, but healing would make them significantly slower at doing gens, so it's a lose-lose situation regardless. This perk is kinda weird, i would say it's unironically even more niche than Distressing. Literaly just a single killer who can use it effectively (technically Dredge's nightfall tied to injuries, but effect is significantly less dramatic and he is M1, so sloppy would do just fine and more reliable).
