I have always had an issue with this perk combo. Braindead teammates run over and loop the killer around the pallet I’m down at and inevitably throw it to my immeasurable disappointment (and anger if it happens enough times to make me punch a hole in something, or to punch my metal 128oz Yeti thermos drink thing across the room).
Or when a teammate runs over to pick me up and throws the pallet because they didn’t rebind their controls so heal and throw pallet are different buttons. (Assuming they are console players)
Very infrequently, when a teammate runs over and throws the pallet, I just DC and walk away.

Those hits were probably not because of the servers. There's a new bug where fast vaults don't place you far enough from a window to avoid being hit.
