I was using Forced Penance. It's really strange, it counts some hits as protection hits even though the survivor "protected" is like 12 meters the other direction.

9:25 to answer everyone’s question about Forced Penance, the way it works on Legion is a little weird. It makes sense though.
So, when you’re chasing a survivor in frenzy and you go for a chain hit, sometimes the chase with the previous survivor won’t end right away when you go for a new one. The game will think you’re chasing both of them. And since you’re hitting another survivor while the game thinks you’re still chasing the previous survivor (who is injured), the game will register it as a protection hit and activate FP.
