Evan's lore is not the best, but I do like the some of the details through cosmetics and tomes. The hooks piercing his back was caused by the Entity, since he resisted the will of the Entity and did not want to kill Survivors. The perk, "Agitation" is a nod to this, since the hooks would cause him pain, so we would hurry up the sacrificing process by carrying Survivors to hooks faster; it's why his mask is on the icon, he does not want to do it but he is forced to take the role and persona the Entity gave him.
"he loved his dad" is not really true he didn't like being controlled, he wanted to breck away from his fathers control, he wanted freedom, and when he did he was scooped up by the entity, who further wanted to control him, that's why he resisted that's why he was scared by the entity. Also while he was the first killer in DBD in the lore there is no 'first killer' because the entity exists outside of time.
