My thought are that Otto conducted this ritual over and over again, filling that island with blood, madness and negative thoughts and feelings. The whole thing about positivity and blissfulness was some sort of emotional plug, used to create some sort of pressure on the victims minds, like a volcano full of madness about to erupt, and when it inevitably erupted the slaughter started. The end result was to create the Dredge, who formed out of this victims limbs, body parts, blood and souls. We can think of it as some sort of Legion, created by different tormented souls that feed on negativity and evil thoughts. And yes, there were even children who were exposed to that madness and gore. If you look carefully, in the Dredges back you can spot a toddlers arm.
