It's actually not a bug for lery's to have two basements. It's a test by BHVR implemented in a shadow update for some of the more bigger maps to sometimes spawn two basements. It's intended

I keep getting two basement spawns on this map. Maybe one of the basements works?
That last part is probably just the entity blocker being on a gen, judging from your footage. Otz did a video where he tried to do 7 gens at once and one of the attempts failed with none popping because grim embrace was proced – every gen was blocked.
