I'm a DBD player I can explain some things here. The default Unknown skin is a male. The skin in the video is of an alternate cosmetic that's supposed to be a female cheerleader. There is also an old woman, and multiple recolors of the male/female skins. The way cosmetics work for killer, you have a head piece, body piece, and a weapon. The different heads have different voices, so the male is male, the female is female, and the old woman is an old woman. It's not actually male or female, it just mimics the voices of it's victims. Also yes you can make a "trans" Unknown by using a male head on the female body or vice versa.
